Résumé : L'inauguration de la salle des urgences est interrompue par une arrivée massive de victimes suite à un accident de train. L'administratrice de l'hôpital, Sharon Goodwin et son personnel font face à une situation d'urgence les poussants à leurs limites dans le triage et le traitement des patients. Tous les médecins sont sur le pont : le nouveau chirurgien traumatique Dr Connor Rhodes, la pédiatre Natalie Dr Manning , le troisième année de résident Dr. Will Halstead , un ancien militaire Ethan Dr Choi , l'infirmière Avril Sexton et le psychiatre Dr. Daniel Charles doivent travailler ensemble pour sauver des vies . Pour compléter l'équipe, on retrouve l'étudiante en médecine Sarah Reese , qui doit apprendre rapidement les dures leçons de la vie des urgences.

Will Halstead: Maggie, where we going?
Maggie Lockwood: Go to Trauma 4.
Ethan Choi: Maggie, I need the X-ray here.
Maggie Lockwood: I got you.
April Sexton: Crossing.
Will Halstead: Whoa, whoa, hold it, hold it… Watch out!
Sharon Goodwin: Katrina, how are all these walk-ins making it through? Let's set a perimeter.
Natalie Manning: Let's get her into Treatment 2.
Ambulance
Connor Rhodes: He keeps biting down. I can't get the blade in. Look, you bag him. I'll do CPR. Come on. We're almost there. It's not time yet.

Sharon Goodwin: Trigger all-hazard disaster plan.
Woman: Yes, ma'am.
Sharon Goodwin: Page to Phone Tree.
Woman: Right away.
Connor Rhodes: 28-year-old male, crush injuries, severed artery, massive blood loss.
Maggie Lockwood: Trauma Three just opened.
Connor Rhodes: Got a tourniquet on his right leg. Tried to intubate but couldn't get his jaw open. He lost his pulse on the ride.
Will Halstead: Put him on our monitor and rapid transfuser. Grab 4 units of O-neg, and start with a round of Epi.
Connor Rhodes: Already hit him with the Epi times two. Still no pulse. He's in fib.
Will Halstead: Let's get him in the bay. I got it from here, man.
Connor Rhodes: Nope, he's mine.
Will Halstead: Okay, who the hell is this guy?
Paramedic: Says he's a doc.
Will Halstead: Get off the gurney, Doc. I'm Senior Resident in this ER.
Connor Rhodes: Yeah? Well, I'm your new trauma fellow.
Maggie Lockwood: Dr. Halstead!
Hallway
Paramedic: Female, early 20s, head trauma. GCS 3, unresponsive in the field.
Maggie Lockwood: Trauma 2 just opened.
Will Halstead: Still GCS 3. We got to change that king airway to a formal ET tube.
Ethan Choi: I'll get a line in the groin.
Treatment 2
April Sexton: Where's your line?
Sylvie Brett: Left AC, obvious femur fracture. Good distal pulse.
April Sexton: It's blown.
Natalie Manning: Sweetheart, we're going to take good care of you.
Rachel Lewis: You have to go back for my daddy! I love my daddy!
Natalie Manning: You know anything about that?
Chout: Pretty sure her father was loaded. Must've been sent to Sinai or County.
April Sexton: Okay, sweetie, look, you see this lady right here?
Rachel Lewis: Uh-huh.
April Sexton: Her name is Maggie. She can find anybody, anywhere, okay? I promise she'll find your daddy for you.
Maggie Lockwood: Better believe I will, hon.
Trauma 3
Connor Rhodes: Hold compressions. You are?
Sarah Reese: Sarah Reese, fourth year med student.
Connor Rhodes: Can you do a cordis in the groin?
Sarah Reese: Mm-hmm.
Connor Rhodes: Get a left femoral in there.
Sarah Reese: Yes, sir.
Connor Rhodes: There, you're sterilized. Now stick it.
Nurse: He's a patient, not a pin cushion.
Sarah Reese: I can't find a vein.
Connor Rhodes: All right. Shock to 120. Clear.
Nurse: No pulse.
Connor Rhodes: Again. 200. Clear.
Nurse: I have a pulse. Sinus tach at 112.
Connor Rhodes: And... He's back. Get a BP and keep giving him blood. Send a rainbow of tubes and a type and cross. Get his blood gas and get him to the OR.

Rachel Lewis: Did you find my dad yet?
Natalie Manning: We will, sweetheart. Don't you worry. All right, now we're gonna put something in your arm to give you medicine. It's just gonna be be a little pinch, okay?
Rachel Lewis: No, no, no!
April Sexton: Make a fist for me, honey.
Natalie Manning: Squeeze my hand. Look at me. Squeeze my hand.
April Sexton: Gonna count to three.
Natalie Manning: Look at me. One, two, three.
Rachel Lewis: Ow.

April Sexton: There you go.

Will Halstead: No gag reflex. Didn't even need sedation. Not good.
Ethan Choi: We got another problem. This woman's pregnant.

Connor Rhodes: Yeah, I'm fine.

Paramedic: 20-year-old male, cystic fibrosis. Complaining of chills, laboured breathing. Temp's 98.

Maggie Lockwood: Respiratory rate is 26. Let's take him to Treatment 6.
Sharon Goodwin: All our ER docs are busy. Can you take care of our friend?
Connor Rhodes: No problem.

Connor Rhodes: Deep breath.
April Sexton: Hey, Jamie.
Jamie Delson: Maybe today... Wasn't such... A good day to drop in.
April Sexton: Oh, we always got time for you.
Connor Rhodes: Let's get a CBC, a CMP, Coags, a chest X-ray, and an ABG. Now, Mr. Dilson, I see that you're use inhaled antibiotics, Tobramycin, Albuterol. You wear a vibration jacket at night and use a humidifier.
Jamie Delson: Yeah.
Connor Rhodes: That's all good.
Jamie Delson: I caught... A cold.
April Sexton: A whopper.
Connor Rhodes: Well, we got to make sure that it doesn't turn into pneumonia. You've had a few episodes.
Jamie Delson: A few means... Three. I've had... More.
Connor Rhodes: Well, we're going to take good care of you here, okay? You're at Northwestern?
Jamie Delson: Yeah.
Connor Rhodes: Well, we better get you out of here quick so you don't miss those mid-terms.
April Sexton: Make a fist.
Hallway
Natalie Manning: Hey, who's my next patient?
Maggie Lockwood: The Red Tags have all been handled. Why don't you sit down for a minute?
Natalie Manning: You know, exercise is good for pregnant women. Come on. Next patient.
Maggie Lockwood: All right. Treatment 1.
Natalie Manning: Thank you.
April Sexton: Crib party Sunday.
Natalie Manning: Crib party?
April Sexton: Maggie and I are gonna come by, help you put the baby's crib together.
Maggie Lockwood: I'll bring the Allen wrenches and deep dish from Kitchen 17.
Natalie Manning: You guys do not have to do that.
Maggie Lockwood: Not if you think that you're gonna do the single-mom thing all by yourself, not with us around.
April Sexton: Takes a village.
Natalie Manning: Thanks, guys. I don't know what I'd do without you.

Sam Abrams: Why am I putting in this bolt? Where's my good-for-nothing resident?
Will Halstead: Put in her 12 hours. Had to knock off.
Sam Abrams: These new rules... Ridiculous, and bad for medicine. "Oh, I'm so tired. Oh, I can't work." Everybody's so damn spoiled, the whole generation.
Beth Donahue: Gina? Gina! Oh, my God.
Maggie Lockwood: Mrs. Donahue...
Beth Donahue: What happened to her?
Maggie Lockwood: Dr. Abrams, Dr. Halstead, this is Mrs. Donahue, the baby's mother.
Will Halstead: Thanks, Maggie. You're the baby's mother?
Beth Donahue: Biological mother. My husband Joel and I, he's... He's at work. He's on his way here. We're the parents. Gina's our surrogate.
Sam Abrams: Opening pressure's 18. I'm done.
Beth Donahue: Look at her. Is she going to be okay... And the baby?
Will Halstead: Baby checks out fine.
Beth Donahue: Oh!
Will Halstead: Healthy heartbeat. Amniotic sac and placenta are intact. Gina, though, I'm afraid, suffered a traumatic head injury and so far, she's unresponsive… We need to contact her family, if you have any information.
Beth Donahue: My husband and I are pretty much her family. She... She's been living with us since before the embryo was implanted.
Will Halstead: Okay.
Beth Donahue: She's gonna get better, isn't she? She's gonna be okay?
Will Halstead: I can't promise anything.
Beth Donahue: No, I can't believe this. What about the baby? It's too soon to deliver, isn't it? She's only 24 weeks.
Will Halstead: Yes, the baby needs more time.
Beth Donahue: What does that mean? Are we gonna lose our child, too? Oh, my God.
Will Halstead: No, Mrs. Donahue. I'm not saying that, and I'm not giving up on Gina, but if the worst did happen, it might be possible to keep her on life support long enough for the foetus to become viable.
Beth Donahue: So... So what, Gina just becomes... A human incubator?
Will Halstead: We're not there yet.

April Sexton: Dr. Rhodes, I thought you could use these.
Connor Rhodes: Thank you.
April Sexton: How'd that happen?
Connor Rhodes: Must have snagged it on the wreckage.
April Sexton: You know, we do have a couple of doctors around here. I could hook you up.
Connor Rhodes: I don't need one, but I could use a nurse though. Come be my other hand.
April Sexton: Oh, nice. You closed that like a plastic surgeon. Where'd you pick that up?
Connor Rhodes: Riyadh. Spent a year there after my residency. Saudis paid me well not to leave unsightly scars. Could you pinch those two sides together?
April Sexton: What about a tetanus shot?
Connor Rhodes: No, I am current, April, uh... April Sexton. English? Irish?
April Sexton: My family's from Brazil. Our real last name is Sanguessuga, but my parents thought Sexton sounded more American, and people could actually pronounce it. Where're you from?
Connor Rhodes: Chicago. Born and bred. Been away for a while.
April Sexton: Welcome home.

Maggie Lockwood: Two and four. Three is headed to the OR.
Sharon Goodwin: All righty.
Daniel Charles: Who's young Rambo over there?
Sharon Goodwin: Hmm? Oh, that's Connor Rhodes, the new trauma fellow.
Daniel Charles: Mm-hmm. He's gonna light up some estrogen receptors around here.
Maggie Lockwood: I'll say. He lit mine.
Sharon Goodwin: Well, that's all I need.
Daniel Charles: Where's Jamie again?
Maggie Lockwood: Treatment 5… A dream.

Natalie Manning: Your daughter's stable. She has a pretty serious leg fracture, but she's okay, just a little shaken up. The orthopedic resident is on his way to put a cast on her.
Mrs Lewis: And what about my husband? I can't get any information...
Natalie Manning: We made some calls. Your husband's down the road at Sinai. They were able to stabilize him, but his injuries are serious. The last word I had, he was headed into surgery.
Mrs Lewis: How is this happening? They were just going on a father-daughter day trip. She wanted to go to the zoo. What...
Natalie Manning: Ms. Lewis, the best thing you can do right now is just be there for your daughter. We'll keep checking on your husband, okay? All right.

Ethan Choi: There. You can see her feet and a hand. Everything looks good, Mr. and Mrs. Donahue.
Joel Donahue: Oh, thank you for doing this. We just needed to see her again.
Beth Donahue: She... She just squeezed my hand. I swear, I felt it.
Ethan Choi: Her reflexes are still intact. Movement can occur, but it's not purposeful.
Beth Donahue: No, she... She just did it again, right when you wiped her stomach. She responded.
Joel Donahue: Tell me that wasn't purposeful.
Will Halstead: Dr. Choi, let's get a repeat CAT scan.

Jamie Delson: You haven't seen... Even one?
Daniel Charles: Hand to God, J, I swear I keep meaning to. I just... You need another blanket here, buddy. How many "Fast and Furious" movies are there anyway? I mean, seriously.
Jamie Delson: Seven.

Jamie Delson: Yeah.

Connor Rhodes: Pleasure. Thank you. You two know each other?

Connor Rhodes: I'd like to intubate you, put you to sleep, get you on a ventilator. You're getting fatigued. This would just allow you to rest so that the antibiotics can do their job.
Jamie Delson: I've been intubated eight times. I think I'll skip it.

Natalie Manning: Hey… Well, there's something I don't often see... Will Halstead looking stumped.
Will Halstead: Why is it brain injuries are never like you read in the textbooks? My pregnant TBI patient, bilateral contusions. She was a GCS 3. And now, all of a sudden, she's exhibiting movement, and she's improved to a 6.
Natalie Manning: Well, that's good news.
Will Halstead: Yeah. The baby's parents will be happy. Oh, she's a... She's a surrogate.
Natalie Manning: Oh. Wow. I don't know how they do that. Carry a baby and then give it up? I mean, I know it brings a lot of joy to other people, but still... Thanks.
Will Halstead: Yeah.
Natalie Manning: You meet the new guy yet, Rhodes?

Connor Rhodes: We're keeping him cool to prevent fever. Come on.

Treatment
Beth Donahue: "Still, he was determined to make it." With an extra shovel of coal, he started up the hill. The little train had never faced" such a long uphill climb before. He was glad that he was red" and that none of the other trains could see him there."
Will Halstead: Mrs. Donahue? This is my colleague, Dr. Manning.
Natalie Manning: Hi.
Joel Donahue: We, uh, we read to the baby every night so she'll recognize our voices.
Natalie Manning: I do that, too.
Will Halstead: Sherry?
Nurse: Dr. Halstead?
Will Halstead: Need to blow down her CO2, take her off the vent and bag her, give her a dose of Mannitol, and increase her Dopamine drip to 12.
Nurse: Right away.
Beth Donahue: Doctor?
Will Halstead: Gina's intracranial pressure is rising.
Joel Donahue: What does that mean?
Beth Donahue: You said that she was getting better.
Will Halstead: Well, she is, in that her function was improving, but the swelling of her brain is getting worse.
Natalie Manning: We're gonna try to control it with medication.
Beth Donahue: And what if the medication doesn't work?
Will Halstead: Then she'll require surgery. The procedure's called a "decompressive craniectomy."
Beth Donahue: Would that be dangerous to the baby?
Will Halstead: There is always a risk for the foetus, and for the mother as well, so, yes.
Natalie Manning: That's why we hope the medication does the trick.
Hallway
Connor Rhodes: Dr. Charles, Jamie has pneumonia, and if there's any hope of beating it, I need to get him on a ventilator now.
Daniel Charles: You heard what he said. He doesn't want that.
Connor Rhodes: Well, we're past what Jamie wants.
Daniel Charles: No, you know what, Doctor? We're actually not past what Jamie wants. Kid has been so brutalized his entire life. He has a right to say no.
Connor Rhodes: Intubating Jamie is the only hope we have of keeping him alive.
Daniel Charles: What's that gonna give him? A week? Ten days?
Connor Rhodes: It could be six months, a year, more, and in that time, anything could happen. New treatments are coming along every day. Now, is he on the list?
Daniel Charles: Right at the top, but a match is highly unlikely. He has HLA antibodies from an islet cell transplant.
Connor Rhodes: Well, have you spoken to his next of kin? Have they weighed in on this?
Daniel Charles: There isn't anybody. Dad took off years ago. His mom's an alcoholic, bounces in and out.
Connor Rhodes: Look, Dr. Charles, you know what's gonna happen. Jamie will lose consciousness, and as his doctor, I'm gonna put him on a ventilator, but by then it may be too late, and there won't be any more choices no matter what anybody wants.
Treatment 3
Sarah Reese: I'm Sarah Reese. I'm a fourth year medical student. Um, Dr. Manning sent me in to check on Rachel before we sign her out… This is for you.
Rachel Lewis: Oh, thanks. What's his name?
Sarah Reese: Why don't you give him a name? I need to... Talk to your mom. So, about your husband...
Mrs Lewis: What?
Sarah Reese: No, no. He's okay. He's still in surgery. The OR nurse says it's going well. As soon as we hear something, I'll let you know. Rachel? Rachel, can you hear me?
Mrs Lewis: What is wrong? What's happening?
Sarah Reese: I can't find a pulse.
Mrs Lewis: Oh, my God.
Voice: Code Blue, Treatment 3. Code Blue, Treatment 3. Code...
Sarah Reese: A little help here!

Ethan Choi: You're not compressing the heart, Sarah. You gotta go harder.
Sarah Reese: But she's so small. I don't want to...

Natalie Manning: What happened?
Ethan Choi: She arrested. We got her back.
Natalie Manning: I can't get an IV. Hand me a drill. I'll get an IO line. I got a line. All right, let's get her a CT/PE study and up to PICU. Mom, you can come with us.
Ethan Choi: Good job.
Sarah Reese: I-I just... I just broke a little girl's ribs.

Daniel Charles: You know more about CF than most doctors.

Maggie Lockwood: Mariana? Hi. I'm Maggie. This... This is Jorge's. Entiendes?
Mariana Acosta: Yes.
Maggie Lockwood: Inside... I found this. "From J.M. to M.A." You're not his girlfriend. You're his fiancée.

Sharon Goodwin: Natalie, what happened with the little girl?
Natalie Manning: She had a cardiac contusion. Bruising caused her to arrest. She's good now, but it was a real scare.
Sharon Goodwin: Thank you.
Natalie Manning: Yeah.

Will Halstead: Your comatose guy... Seems like you'd want to put him on the hypothermic protocol and protect his brain.
Connor Rhodes: Well, I just gave him 20 units of blood. His pH is only 7.2, and he's too cold to make a clot, so I didn't see how the hypothermic protocol would help him.
Will Halstead: Hey, you're the fellow. I'm just a resident. Heard you're local. Me, too. Where'd you grow up?
Connor Rhodes: Lake Shore Drive.
Will Halstead: Nice. Well, I doubt we have any friends in common… Canaryville.
Connor Rhodes: Oh.
Will Halstead: Where'd you go to med school? Johns Hopkins? Stanford?
Connor Rhodes: No. I went to UAG.
Will Halstead: Where's that?
Connor Rhodes: Guadalajara.
Will Halstead: Didn't see that coming. Got to go.

Will Halstead: Her ICP is unresponsive?
Natalie Manning: Her CPP is 40.
Will Halstead: Sherry, get me Dr. Abrams.
Joel Donahue: You... You're planning to do the surgery?
Beth Donahue: What about our baby?
Will Halstead: The meds we've been giving Gina haven't been effective. Her intracranial pressure, the brain swelling, is getting to a dangerous level, and we shouldn't wait any longer.
Beth Donahue: I'm not sure we can consent to that.
Natalie Manning: It's not really a matter of consent.
Joel Donahue: Well, actually, Doctor, it is. We have a surrogacy contract with Gina. A rider to that is a Medical Power of Attorney, giving us the right to make decisions on Gina's behalf.
Beth Donahue: I don't see how we can sign off on a procedure knowing it will endanger our child.
